Name Change

A name change may not be sought for any fraudulent purpose or if it will infringe upon the rights of another. You must file the name change in the Circuit Court of the county in which you live.

Petitioners who desire to change the name of their child or ward must obtain consent of both parents to change a child’s name. Make sure you select the Name Change Petition for a minor.  You must come to the Clerk’s Office for the Name Change Order for a minor.

Instructions for Name Change Procedure(Adult):

  1. Fill out the Petition and Order
  1. Provide copy of a birth certificate in the current name and marriage license when applicable.
  2. Filing Fee of $39.00 must be cash or check payable to: John Barett Chappell, Jr; Clerk
  1. Filings may be submitted in Person or by Mail

  1. Normal processing times will vary between 2 and 4 weeks.  Once the Judge signs the Order, we will mail a certified copy of the Order to you.

2. Both Parents must sign and notarize the application. 

3. Provide copy of a birth certificate for the minor and a copy of the parent’s birth certificate and driver’s licenses in the current name and marriage licenses if applicable

4. Filing Fee of $39.00 must be cash or check payable to: John Barrett Chappell, Jr.; Clerk

5. Filings may be submitted in Person or by Mail.
